Minimum Qualifications:

  • Master's degree in nursing, healthcare administration, public health or related field required or in active pursuit and obtained within 2 years from date of hire or transfer.
  • Graduate of an accredited school of nursing required.
  • Current New Jersey registered nurse license.
  • Valid New Jersey driver's license, registration and insurance are required.
  • BLS required.
  • Five years of healthcare operations management experience, preferably in PACE, long-term care, ambulatory care or other licensed community-based service.
  • Ten years of nursing experience.

POSITION SUMMARY
Under the supervision of the Executive Director, has overall responsibility for design, implementation, performance and evaluation of all clinical services provided in the Center and at home by non-primary care provider licensed and credentialed members of LIFE Connection staff, including nursing, physical therapy, occupational therapy, registered dieticians and certified home health aides.

Ensures all clinical services are planned and provided in compliance with relevant laws and regulations and established standards of care and best practice models. Ensures all clinical staff have and maintain required licensure and certification as required for their position. Ensures all clinical staff maintain demonstrated competence for their full scope of practice. Works collaboratively with Medical Director to ensure effective collaboration between primary care providers and all other care disciplines for optimum participant care.

Ensures all clinical members of the interdisciplinary team consistently practice in a collaborative manner within their established scope of practice. Serves as a member of LIFE Connection leadership team. Serves as Director of Nursing or assigns in writing a qualified designee who shall be in the LIFE facility during the hours of operation. Remains current with clinical practice updates and innovations to ensure clinical services at AtlantiCare LIFE Connection consistently meet or exceed all professional standards.
 
QUALIFICATIONS

EDUCATION:
  • Master's degree in nursing, healthcare administration, public health or related field required or in active pursuit and obtained within 2 years from date of hire or transfer.
  • Graduate of an accredited school of nursing required.

LICENSE/CERTIFICATION:
  • Valid New Jersey nursing license required.
  • BLS required.
  • Valid New Jersey driver's license, registration and insurance are required.

EXPERIENCE:
  • Minimum of five (5) years of healthcare operations management experience, preferably in PACE, long-term care, ambulatory care or other licensed community-based service.
  • At least 10 years of nursing experience required.
  • At least 2 years of senior healthcare leadership experience required.
  • Experience with an interdisciplinary model of care in managed care, hospice, home health or PACE required, working directly with other healthcare disciplines, including primary care, pharmacy, social work and physical and occupational therapy.
  • Minimum of one year of experience working with the frail elderly population.
  • Proven experience in development and management of collaborative, innovative service models preferred.
  • Healthcare informatics experience preferred.
  • Experience with an electronic health record required.
  • Minimum 7 years of experience, preferably in serving the aging population.
  • Proficiency in clinical applications preferred at time of hire. Incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.

PERFORMANCE EXPECTATIONS
Demonstrates the technical competencies as established on the Assessment and Evaluation Tool.

WORK ENVIRONMENT
Potential for exposure to blood-borne pathogens, along with infectious diseases. The work settings are in an adult day health center and primary care clinic environment with moderate noise levels and controlled temperatures.  Requires manual and finger dexterity and hand-eye coordination. Ability to understand department equipment and lift up to 30 pounds using appropriate body mechanics.
