Position Summary:
The care manager will possess a bachelor's degree in one of the helping professions, such as social work, human services, counseling, psychiatric rehabilitation, psychology or criminal justice. Staff with a bachelor's-level degree in a field other than a helping profession (as listed above) shall have a minimum of two (2) years of professional experience in a community behavioral health setting. Certification as a psychiatric rehabilitation practitioner may be substituted for one year of the required two years of professional experience. The care manager will demonstrate warmth, hopefulness and enthusiasm in their work on behalf of consumers and will have a personal philosophy that is person-centered and embodies wellness, recovery and resiliency. The care manager will work under the supervision of a physician, psychiatrist, licensed psychologist, advanced practice nurse, registered nurse or master's degree CSS staff. Care managers will perform the duties and responsibilities listed below provided they are authorized to perform the duty or responsibility under their professional licensure, certification or credentialing standards.
Duties and Responsibilities:

Contributing to the development, implementation, monitoring and updating of the individualized rehabilitation plan pursuant to NJAC 10:37B-2.4.

Performing therapeutic rehabilitative skill development pursuant to NJAC 10:37B-4.2.

Offering services appropriate to meet the consumer's needs pursuant to NJAC 10:37B-4.6.

Performing crisis intervention pursuant to NJAC 10:37B-4.4(a)24.

Coordinating and managing services as directed.

Engaging with program participants in a warm, empowering manner.

Exhibiting a person-first approach in all interactions and interventions.

Providing skills-building education in daily living skills.

Assistance with housing acquisition and preparing for move-in, including furnishing, utilities, etc.

Assisting individuals in developing Wellness and Recovery Action Plans (WRAP) and Psychiatric Advanced Directives (PAD).

Explanation to consumer resident of their rights and procedures for removal/discharge from housing.

Providing assistance with linkage to mental health and substance abuse services.

Encouraging and facilitating family involvement.

Providing assertive care management and supportive counseling.

Initiating linkage or reconnection with clinical services.

Providing interventions to stabilize community living situation.

Researching transportation options and coaching consumers on utilizing transportation resources that meet their needs.

Ensuring linkage with entitlements and community resources, including vocational and co-occurring drug and alcohol services.

Planning and coordinating recreational activities.

Performing housing inspections as necessary.

Working a flexible schedule to accommodate resident needs.

Performing on-call responsibilities as assigned.

Documenting all consumer, family and case management activities.

Completing documentation within one calendar day of service delivery.

Maintaining valid New Jersey driver's license in good standing. Transports consumers as needed.
