The PACE Medical Director will have the opportunity to oversee all facets of our “nursing home without walls”, from intake to care planning and through end-of-life care. This is an exceptional opportunity to lead an interdisciplinary care team and coordinate effective care plans for LIFE Connection participants to receive ongoing comprehensive care services from the comfort of their homes, rather than an extended care facility. Here, you can maximize your program management abilities by building and directing a multifaceted network, including medical specialists, primary care providers and geriatricians. We’re seeking a compassionate care provider who will develop and initiate policies and procedures that lead Quality Assurance Performance Improvement (QAPI) initiatives while inspiring the highest standards for patient safety, sustained health and quality of life.
The successful candidate should be a BC/BE IM, FM or Geriatric Medicine physician with supervisory experience, preferably within a program or department focused on providing long-term care for the elderly population. We seek an accountable, data-driven practitioner who excels at translating coding data, quality scores and admission-rate data into benchmark programs; making informed quality assessments; performing utilization management tasks; and recommending performance improvement plans. Advanced knowledge of coding procedures and documentation in Medicare Risk Adjustment Payment Systems is essential to this role. This is highly interactive position, so the candidate must have strong interpersonal skills that will enhance their ability to conduct PACE enrollment visits, educating participants and their families about the benefits of the program and managing cross-functional teams remotely and face-to-face.
