POSITION SUMMARY
Under the direction of leadership, the discharge planner is responsible for coordinating continued care and establishing community-based resources to be utilized for clients who are completing residential care at John Brooks Recovery Center (JBRC).
This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.
QUALIFICATIONS

EDUCATION: Bachelor's degree in a related field required.

EXPERIENCE: Minimum 1 year of experience in collection of data in an interview setting required.
PERFORMANCE EXPECTATIONS
Demonstrates the technical competencies as established on the Assessment and Evaluation Tool.
WORK ENVIRONMENT
This position requires repetitive motion, hearing, pushing, reaching, reading, seeing, speaking, standing and walking a majority of the time. Spends time with such equipment as a computer terminal, fax machine, copier and telephone.

REPORTING RELATIONSHIP
This position reports to department leadership and supervises incumbents as assigned.
