The community care navigator is part of the healthcare team working to improve HIV/AIDS clients retained in care and adherent to antiretroviral therapies by conducting community-based and in-home outreach.
The community care navigator provides rapid HIV testing, social support, informal counseling, outreach activities and intensive navigation support to help clients address needs that make it challenging to stay healthy, including transportation, housing, insurance benefits, mental health, substance use, stigma and food, and in navigating complex care and support systems.
The community care navigator demonstrates understanding of community resources and communicates and collaborates effectively with community-based organizations.
The community care navigator improves treatment adherence by providing educational sessions on topics such as HIV/AIDS, HIV viral load life cycle and medication side effects and addressing barriers to care. Finally, the community health worker supports the HIV care continuum by attending all required training and meetings related to the program.
This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.
QUALIFICATIONS

EDUCATION: High school diploma required. Associate's degree preferred.

LICENSE/CERTIFICATION: Active driver's license.

EXPERIENCE: Minimum 1-2 years of experience working in a healthcare setting. Excellent written and verbal communication skills required. Bilingual preferred.

Demonstrates ability to work with diverse populations.

Proficiency in clinical applications preferred at time of hire. Incumbents within position will be trained appropriately and then skills will be required for this position within 30-60 days from date of hire.

Demonstrates the competencies as established on the Assessment and Evaluation Tool for this position.
WORK ENVIRONMENT
Potential for exposure to the hazards and risk of the hospital environment, including exposure to infectious disease, hazardous substances and potential injury. This position requires sitting, standing, walking, stooping and crouching a majority of the workday. Works with such equipment as a computer terminal, fax machine, printer and copier.
REPORTING RELATIONSHIP
This position reports to the department director, manager or nursing leadership in other departments within the organization.
