POSITION SUMMARY
Under the direction of leadership, the Case Manager, Substance Abuse conducts assessments at the "Single Point of Entry" in Atlantic City. The incumbent is expected to work in coordination with and enlist the services of specialized "Single Point of Entry" staff, such as Mental Health, Traveler's Advice or Outreach in order to help the homeless individual implement a plan that will move them out of a state of homelessness. As requested by the program director, the Case Manager, Substance Abuse will represent the "Single Point of Entry" on homeless-related committees in the community and conduct outreach activities within the parameters and scope of the program.
This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.
QUALIFICATIONS

EDUCATION: Bachelor's degree in a related field required.

LICENSE/CERTIFICATION: CADC required. Valid driver's license required. Valid registration and insurance required.

PERFORMANCE EXPECTATIONS
Demonstrates the technical competencies as established on the Assessment and Evaluation Tool.
WORK ENVIRONMENT
This position requires repetitive motion, hearing, pushing, reaching, reading, seeing, speaking, standing and walking a majority of the time. Spends time with such equipment as a computer terminal, fax machine, copier and telephone. Occasional driving also required.
