Position Summary:
The care manager will possess a bachelor's degree in one of the helping professions, such as social work, human services, counseling, psychiatric rehabilitation, psychology or criminal justice. Staff with a bachelor's-level degree in a field other than a helping profession (as listed above) shall have a minimum of two (2) years of professional experience in a community behavioral health setting. Certification as a psychiatric rehabilitation practitioner may be substituted for one year of the required two years of professional experience. The care manager will demonstrate warmth, hopefulness and enthusiasm in their work on behalf of consumers and will have a personal philosophy that is person-centered and embodies wellness, recovery and resiliency. The care manager will work under the supervision of a physician, psychiatrist, licensed psychologist, advanced practice nurse, registered nurse or master's degree CSS staff. Care managers will perform the duties and responsibilities listed below, provided they are authorized to perform the duty or responsibility under their professional licensure, certification or credentialing standards.
Duties and Responsibilities:

Contribut to the development, implementation, monitoring and updating of the individualized rehabilitation plan pursuant to NJAC 10:37B-2.4.

Performs therapeutic rehabilitative skill development pursuant to NJAC 10:37B-4.2.

Offers services appropriate to meet the consumer's needs pursuant to NJAC 10:37B-4.6.

Performs crisis intervention pursuant to NJAC 10:37B-4.4(a)24.

Coordinates and manages services as directed.

Engages with program participants in a warm and empowering manner.

Exhibits a person-first approach in all interactions and interventions.

Provides skills-building education in daily living skills.

Assists with housing acquisition and preparing for move-in, including furnishing, utilities, etc.

Assists individuals in developing Wellness and Recovery Action Plans (WRAP) and Psychiatric Advanced Directives (PAD).

Explains to consumer resident their rights and procedures for removal/discharge from housing.

Provides assistance with linkage to mental health and substance abuse services.

Encourages and facilitates family involvement.

Provides assertive care management and supportive counseling.

Initiates linkage or reconnection with clinical services.

Provides interventions to stabilize community living situation.

Researches transportation options and coaches consumers on utilizing transportation resources that meet their needs.

Ensures linkage with entitlements and community resources, including vocational and co-occurring drug and alcohol services.

Plans and coordinates recreational activities.

Performs housing inspections as necessary.

Works a flexible schedule to accommodate resident needs.

Performs on-call responsibilities as assigned.

Documents all consumer, family and case management activities.

Completes documentation within one calendar day of service delivery.

Maintains a valid NJ driver's license in good standing. Transports consumers as needed.
