Position Summary:
The Care Coordinator – Hospital-Based Behavioral Health Services is responsible for providing prompt screening of referrals for behavioral health and substance use services and assist in the coordination of care, discharge planning and linkage to appropriate behavioral health and social services.
Duties and Responsibilities:
Provides prompt and accurate triage and screening of individuals referred.
Provides competent screening and risk assessment for substance use or other behavioral health-related problems.
Provides competent crisis intervention as appropriate.
Makes referrals and completes linkages to appropriate services with appropriately credentialed providers.
Demonstrates knowledge of ARMC/ABH, healthcare systems and community resources.
Guides patient through continuum acting as liaison between multidisciplinary team, physicians, external agencies and family where applicable.
Effectively engages individuals/families in helping relationships using acquired professional skills and an inclusive and person-centered approach.
Demonstrates understanding of patient's behavioral health needs and implications for family support, education and discharge planning.
Effectively communicates financial and social needs/barriers/issues to the multidisciplinary team, physicians and external agencies.
Demonstrates knowledge of privacy practices.
Assists clients in scheduling external appointments as appropriate based on need.
Completes proper documentation in a timely manner.
Provides necessary documentation to external agencies as needed.
Conducts post-discharge follow-up phone calls to patients.
As appropriate to scope of practice, assists in the data collection for each assigned client, including the following:
The relationship of each client's physical status to dependence.
The nature of use of alcohol and/or other drugs.
The intensity of the client's mental preoccupation with using alcohol and/or drugs.
As appropriate to scope of practice, demonstrates a knowledge of the natural history of use disorder.
As appropriate to scope of practice, demonstrates an understanding of the biopsychosocial influences and effects of use disorder.
Proficient in the knowledge and skills described in the core competency curriculum for each assigned age/disability group.
Attends 80% of all scheduled staff meetings.
Minimal Qualifications:
Education: Bachelor's degree in a behavioral-health related field. CADC tracking preferred. Will consider three years of related work experience in lieu of bachelor's degree.
Training: Assessment using DSM5, ASAM, crisis intervention and training/experience in mental health and substance use evaluation or NJ CADC.
Must show proof of valid driver's license.
Preferred Qualification:
Training/Experience: Training/experience in substance use and mental health assessment. Strong knowledge of community resources and healthcare systems. Practical experience in crisis intervention and insurance verification. Good planning and organizational Skills, excellent interpersonal skills and strong verbal and written communication skills.
At least one year of experience in behavioral health field or appropriate internship required.
Understands and supports a trauma-informed system of care.
Experience with illness management and recovery (IMR) and motivational interviewing (MI) preferred.
